KGMU OPD Registration 2026: New Patient, UHID & Follow-Up Guide
KGMU OPD registration is the patient-record and reporting step that connects an appointment or walk-in instruction with the actual outpatient visit. New patients may need a new KGMU record, while returning patients should preserve the existing UHID/OPD information so follow-up care stays connected with earlier consultations.
KGMU OPD registration 2026: quick details
| Registration detail | Information |
|---|---|
| Outpatient registration charge | ₹50 |
| Validity | Up to 6 months |
| New patient | Create/verify patient record as required |
| Returning patient | Use existing UHID/OPD record where supported |
| Online appointment | Separate from hospital reporting/consultation |
| Registration counters | Multiple counters in New OPD plus facility-specific routes elsewhere |
Appointment, registration, OPD slip and consultation are different steps
Patients often use these terms interchangeably, but they refer to different parts of the visit. Understanding the difference helps avoid duplicate registration or reaching the clinic without the patient record needed for that visit.
| Stage | What it means |
|---|---|
| Appointment | A booked department/date through the available online or hospital route. |
| Registration / UHID | Creation or retrieval of the KGMU patient record. |
| OPD slip / card | The visit document linked with the current registration/department. |
| OPD reporting | Reaching the correct clinic, unit, counter or room. |
| Consultation | The clinical visit with the department/team. |
KGMU new patient registration
Before you start
- Keep the patient’s correct name, age/date of birth, mobile number and address ready.
- Keep the appointment confirmation if booked online.
- Carry a referral letter when the visit is based on a referral.
- Bring previous reports relevant to the reason for referral.
At the hospital
Follow the registration/reporting instruction for the building where your department operates. New OPD has multiple registration counters on the ground floor, but Queen Mary, Lari Cardiology, Dental, Psychiatry and other facilities can have their own patient flow. Do not assume every KGMU patient must report to the same counter.
KGMU returning / old patient registration
Returning patients should keep the previous KGMU UHID, OPD slip or patient number available. The current registration is listed as valid for up to six months, so an existing active record can be important for follow-up visits, test review and continuity with the previous department.
Follow-up checklist
- Previous KGMU prescription.
- UHID / patient number / old OPD slip.
- Written return date or unit/clinic name.
- Investigation reports requested at the previous visit.
- Discharge summary if returning after admission or surgery.
Where are KGMU registration counters?
The New OPD directory lists multiple registration counters on the ground floor. That information is useful for departments using New OPD, but KGMU has several patient facilities with their own workflow. Always match registration with the building used by the department or service.
Find a KGMU patient location
Search by department or building. Results distinguish OPD, department, ward, laboratory, procedure and emergency locations.
KGMU UHID, patient number and OPD slip
UHID / patient identity
The hospital patient identity links visits and records. Returning patients should avoid creating a second record unless the hospital specifically instructs them to do so.
OPD slip
The OPD slip is the visit document generated or issued for the outpatient process. Keep a digital or printed copy where available until the consultation and follow-up are complete.
Lost KGMU UHID or old OPD slip
If an old slip is unavailable, do not invent a new patient identity merely to continue quickly. Keep other information that can help locate the existing record—registered mobile number, previous prescription, appointment number or discharge record—and ask the relevant registration/help desk how the old record can be retrieved.
KGMU registration for patients with an online appointment
An ORS appointment can reduce the work needed before the visit, but the patient may still need to complete local registration verification or reporting after reaching the hospital. Keep the appointment confirmation ready and go directly to the correct patient facility rather than assuming the booking removes every hospital step.
KGMU registration for different hospital buildings
New OPD
Used by many specialty departments. Ground-floor registration counters are listed in the KGMU internal directory.
Queen Mary Hospital
Obstetrics, Gynaecology, ANC, postnatal and maternity-related services use Queen Mary’s own patient pathway rather than the general New OPD route.
Lari Cardiology
Cardiology OPD and cardiac services use the Lari Cardiology complex. A Cardiology patient should not be sent to New OPD merely because it is a KGMU outpatient visit.
Dental
Dental patients should follow the Faculty of Dental Sciences department/clinic route. Multiple dental specialties share the broader Dental Faculty but have different clinics.
Documents to carry for KGMU OPD registration
| Document | Use |
|---|---|
| Appointment confirmation | Shows booked department/date where applicable. |
| Previous KGMU OPD slip / UHID | Links returning patient with earlier record. |
| Photo ID / identity details | Carry where required. |
| Referral | Shows the department requested by the referring doctor/hospital. |
| Previous prescription | Important for follow-up, unit and clinic instructions. |
| Requested reports | For review when the treating team has already asked for them. |
Common KGMU registration problems
Wrong department selected
Correct the department route before joining an unrelated OPD queue.
Existing patient registered again
Ask the registration/help desk whether the existing UHID can be retrieved instead of maintaining duplicate records.
Appointment slip not found
Check SMS, browser downloads, screenshots and the ORS account before creating another booking.
Reached the wrong building
Use the department’s patient-facing location in the Campus Navigator; department office and routine OPD can differ.
KGMU registration validity and repeated follow-up visits
Because the outpatient registration is listed as valid up to six months, patients who return repeatedly for review should preserve the same registration information. This is especially useful when the treating unit asks for tests and then schedules another review within the validity period.
Registration for department-specific follow-up
Follow-up registration should preserve the department and unit/clinic context from the previous consultation. A patient already linked to Orthopaedic Unit III, a Neurology specialty clinic or a Respiratory Medicine unit should carry that information so the next visit does not become a generic new-patient route.
What to do after registration
- Confirm the department/clinic on the OPD document.
- Move to the patient-facing building/floor/room.
- Follow the local token/queue instructions.
- Keep the OPD slip and previous records ready for the consultation.
- Before leaving, save the next follow-up date and any requested investigations.
